Terms Of Service Agreement

The following terms of service (the “Agreement”) is effective

BETWEEN: THE CORE FOUNDATION (The “Service Provider”), a corporation organized and existing under the laws of the state of Utah, with its head office located at:

530 East 500 South

Salt Lake City, UT 84102

AND THE UNDER SIGNED (“The Individual”)

WHEREAS, the “Service Provider” is in the business of development, supply and operation of the

products and educational services related to The Core Foundation X-Ray Certification or CPR certification training programs; and

WHEREAS, this Agreement provides the “Service Provider’s” terms of engagement;

NOW THEREFORE, in consideration of the mutual covenants and agreements herein contained, the

 parties hereto, intending, to be legally bound, agree as follows:

1. SERVICES PROVIDED

The “Service Provider” is prepared to offer the following educational and training services to the “Individual”:

CPR Certification: $75

Our American Red Cross certified CRP program is a hands-on training course. This CPR course is taught be experienced instructors and consists of hands-on training to ensure you exceptional training that sticks in your mind. This class varies between 4-8 hours. 

OR

X-Ray Certification: $89

Our X-Ray certification is taught in a classroom as well as a state-of-the-art x-ray room that is equipped with the most current machines and technology. This course is a total of 4 – 6 hours in length. 

2. BILLING ARRANGEMENTS

Tuition and associated finance fees must be paid in advance of the certification class start date.

IN WITNESS WHEREOF, each party to this agreement has caused it to be executed at The Core Foundation (TCF): 530 East 500 South, Salt Lake City, UT 84102.
